Online registration is now closed. Onsite registration will be available in Kohler. Please print out the registration form below and bring it with you to expedite the onsite registration process. We will also have forms available onsite.
You may also opt to fill out a registration form and mail it together with your payment. Additional forms may be printed for more registrants. Checks should be made payable to Angle Midwest and sent to: Dr. Libby Graham, 403 Linden Avenue, Wilmette, IL 60091. Registration is not complete until payment is received.
The registration form includes selections for banquet meal, activities, and Midwest Component dues.* These dues are to be paid by Active, at-Large, and Senior Active Members ($250), as well as Affiliates ($125), even if you are unable to attend the annual meeting. Dues may be paid without registering for the meeting by mailing your payment directly to Dr. Libby Graham. Senior Retired Members, Candidates, and Guests do not pay dues.
